What peptide therapy is and how it works

Peptides are chains of two to fifty amino acids. The body makes thousands of them as messengers: insulin, the GLP-1 hormone that regulates appetite, growth hormone releasing hormone and collagen fragments are all peptides. Because each peptide binds a specific receptor and triggers a specific response, they can be used as targeted medicines. Several are FDA-approved drugs, including insulin, the GLP-1 medications used in weight management and bremelanotide for sexual health, and many more are prescribed by licensed providers and prepared by compounding pharmacies.

Peptide therapy at Rethink starts with a medical question: what do you want to change, and what do your labs show? Tami then selects a protocol from peptides that a licensed U.S. pharmacy can legally dispense, sets the dose and duration, and teaches you how to take it. Most peptides are given as a small subcutaneous injection at home with an insulin-type needle; some come as nasal sprays, capsules or creams. The FDA regularly reviews which peptides compounding pharmacies may prepare, and the list changes, which is one reason a prescriber reviews the plan.

How it works

How the peptide program works at Rethink

Visits take place in a private suite at 496 Plandome Road in Manhasset. The first visit takes 45 to 60 minutes; check-ins every four to eight weeks are shorter and can be combined with other wellness visits.

Consultation and lab review

Tami takes a full history: goals, sleep, training, injuries, medications, supplements and symptoms. She orders lab work or reviews recent results (metabolic panel, thyroid, hormones, inflammatory markers as relevant) so the protocol addresses what is actually happening. The complimentary consultation covers the approach; labs and medication are quoted separately.

Your protocol

Based on the goal and the labs she selects one or two peptides, the dose, the schedule and the duration, usually a cycle of eight to sixteen weeks, and writes it down. The prescription goes to a licensed pharmacy, and the peptide is shipped to you or picked up at the office. Where a peptide is not appropriate or cannot be legally dispensed, she says so and offers alternatives.

Starting the protocol

At the start Tami shows you how to store, measure and inject the peptide with a small insulin-type needle, or how to use a spray, capsule or cream. Most injections take a few seconds under the skin of the abdomen or thigh. She explains what to expect in the first weeks and how to reach her with questions.

Check-ins and cycles

Every four to eight weeks you review sleep, energy, recovery, measurements and side effects, and the dose or the peptide is adjusted. Labs are repeated where they guide the plan. Most protocols run in cycles with a pause and a reassessment, so treatment continues only while it is doing something for you.

Results, timeline and side effects

Peptides work gradually and depend on the goal. Sleep and recovery protocols often show a difference within two to four weeks. Body composition and strength changes take eight to twelve weeks of consistent use together with training and protein intake. Skin and hair peptides show results over two to three months. Improvements are measured at the check-ins in sleep, energy, measurements and, where relevant, repeat labs, and protocols that show no benefit are stopped.

Side effects are usually mild and dose-related: redness or a small bump at the injection site, water retention, tiredness or headaches in the first days, and appetite changes with metabolic peptides. Safety rests on three things: a medical screening, a peptide from a licensed pharmacy with verified purity and dose, and monitoring during the cycle. Rethink does not use research-grade or online-only peptide products.

Who is a candidate for peptide therapy

Peptide therapy suits adults with a clear goal, recovery, sleep, body composition, healthy aging or sexual wellness, who are willing to follow a protocol for several weeks and attend check-ins. It fits well alongside medical weight management, IV therapy and vitamin injections, and it is a reasonable next step for patients whose diet, training and sleep habits are already in order and who want additional support.

Peptides are not prescribed during pregnancy or breastfeeding, with active cancer or a recent history of cancer (relevant for peptides that stimulate growth hormone), with uncontrolled diabetes or thyroid disease, or alongside medications that interact. Do you offer BPC-157?

BPC-157 is one of several peptides the FDA restricted from pharmacy compounding in 2023, so it cannot be prescribed and dispensed legally in the United States at this time and is not part of the program at Rethink. Tami can discuss alternatives for recovery and inflammation that a licensed pharmacy can provide.

How much does peptide therapy cost?

The consultation is complimentary. The monthly cost depends on the peptide, the dose and the cycle length, plus lab work if needed, and is quoted in writing at the first visit. The pharmacy bills the medication separately. Payment plans are available.

Does insurance cover peptide therapy?

No. Wellness peptide protocols are paid privately. Lab work ordered for medical reasons is sometimes covered by insurance; ask at the consultation.

Do I have to inject myself?
