Overview
Platelet-rich plasma, or PRP, is the fraction of your blood that carries platelets and the growth factors they release when tissue needs repair. A small sample is drawn from your arm, spun in a centrifuge to concentrate the platelets several times above normal, and the plasma is injected in a grid of small, shallow injections across the thinning areas of the scalp. The growth factors improve blood supply to the follicles, wake follicles that have gone dormant and prolong the growth phase of the hair cycle, so hairs grow thicker and shed less.
PRP works on follicles that are miniaturized, meaning still present but producing thin, short hairs. That is the pattern in early to moderate androgenetic alopecia, the common hereditary thinning in women and men, and in shedding after stress, illness, pregnancy or weight loss. It does not create follicles where they are gone, which is why treating early gives the best results. PRP is drug-free and uses only your own blood, so it can be combined with topical or oral treatments your dermatologist prescribes.

What it treats
PRP helps most where follicles are weakened but present. A scalp examination at the consultation confirms the pattern, and Tami will say so if a dermatology referral, blood work or another approach should come first.
How it works
A session takes 45 to 60 minutes in a private treatment suite at 496 Plandome Road in Manhasset. New patients begin with a complimentary consultation and scalp assessment.
Tami reviews your medical history, medications, family history of hair loss and how long the thinning has been present, and examines the scalp and the hair shafts. Where the cause is unclear, blood work for thyroid, iron and vitamin D is recommended first. Photographs are taken so that progress can be compared.
A small sample of blood, about the amount of a routine lab test, is drawn from your arm and spun in a centrifuge for about ten minutes. The platelet-rich layer is separated and drawn into syringes. Meanwhile the scalp is cleansed and, if you wish, a topical anesthetic or cooling is applied.
The PRP is injected in a grid of small, shallow injections about a centimeter apart across the thinning areas, with a very fine needle. The scalp is less sensitive than the face, and most patients describe brief pinches and pressure. The injection phase takes 15 to 20 minutes.
Leave the scalp dry for the rest of the day and wash it gently the next morning. Skip strenuous exercise, saunas and harsh styling products for 24 to 48 hours. Sessions are spaced four to six weeks apart, usually three to four in the first series, with a maintenance session every six to twelve months.
Results
Hair grows slowly, so PRP results follow the hair cycle. Shedding usually decreases first, within four to eight weeks of the first session. New growth and thicker shafts become visible at three to four months, and the full result of a series shows at six to nine months. Studies of PRP for androgenetic alopecia report increases in hair density and shaft thickness in most patients after three to four sessions; how much depends on how early treatment starts and on the underlying cause.
Downtime is minimal. The scalp may feel tight or tender for a day, and small bumps or pinpoint bruising at the injection sites resolve within a day or two. You can return to work immediately, and hair can be styled normally the next day. Results are maintained with a session every six to twelve months, because the underlying tendency to thin continues; patients who stop treatment lose the gain gradually over a year or more.
Candidates and safety
PRP suits women and men with early to moderate thinning who still have hair in the affected area, and patients with shedding that has not recovered. It suits patients who want a drug-free option, who cannot or prefer not to take oral medication, and those already using minoxidil or finasteride who want to add to the effect. General health matters, because the platelets come from your own blood: a healthy diet, iron and vitamin D levels and thyroid function all influence the result.
PRP is not recommended for completely bald areas, for scarring alopecias such as lichen planopilaris or for active scalp infection, and it is postponed during pregnancy and breastfeeding. Low platelet counts, blood disorders, anticoagulant therapy, active cancer and uncontrolled diabetes are reasons to decline or to coordinate with your physician first. Smoking and heavy alcohol use reduce the quality of the plasma, and Tami will discuss what to change before the first session. If the examination points to a medical cause, she refers you to a dermatologist before treating.
