What dermal fillers are and how facial balancing works

Dermal fillers are smooth gels of hyaluronic acid, a sugar molecule that occurs naturally in skin and holds water. Injected beneath the skin or along the bone, the gel adds volume, supports tissue that has thinned with age and smooths folds. Rethink uses FDA-approved hyaluronic acid fillers, a category that includes the Juvederm and Restylane families. The product for each area is chosen for its firmness and the way it moves: the lips need a soft, flexible gel, and the chin or jawline needs a firmer one that holds its shape. Hyaluronic acid fillers can be dissolved with an enzyme if needed, which is one reason they are the standard choice for the face.

Facial balancing is the planning approach behind every filler treatment at Rethink. Tami assesses the face as a whole: the proportions of the forehead, midface and lower face, the profile from the side, the symmetry between the two halves and how the features relate to each other. A small chin can make the nose and lips look larger than they are, and flat cheeks can deepen the folds beside the mouth. Treating the cause, often in a different area from the one that first bothered you, gives a more natural result with less product.

Each area has its own product, technique and typical amount. A syringe holds 1 mL. The lips usually take half a syringe to one syringe, and the cheeks, chin or jawline one to two syringes each, split across visits when a plan covers several areas.

How it works

How filler treatment works at Rethink

Filler appointments take 45 to 60 minutes in a private treatment suite at 496 Plandome Road in Manhasset. First visits begin with a complimentary consultation, and treatment can follow the same day when you are ready.

Consultation and facial assessment

Tami reviews your health history, then assesses your face from the front and in profile, at rest and in motion. She explains which areas contribute to the change you want, how many syringes a plan needs and in what order, and what a first visit can realistically achieve. Photographs can be taken for your record.

Numbing and treatment

A topical anesthetic sits on the skin for 15 to 20 minutes, and the fillers themselves contain lidocaine, so most patients feel pressure and a few sharp moments. Product is placed with a fine needle or a blunt cannula, in small amounts, with frequent checks in the mirror. The lips take about 20 minutes, a multi-area plan up to an hour.

Aftercare

Expect swelling for two to three days, most noticeable in the lips, and possible bruising for up to a week. Use cold compresses, sleep with your head raised the first night, and avoid alcohol, strenuous exercise, saunas and firm pressure on the area for 24 to 48 hours. Dental work is best scheduled two weeks away from filler.

Follow-up and staging

A follow-up at about two weeks confirms that the product has settled evenly, and a small addition is made if an area needs it. Plans that cover several areas are staged over two or three visits, four to six weeks apart, so each step is judged before the next. Maintenance is planned by area, from six to 18 months.

Results

Results, how long fillers last and downtime

You see the change immediately, and the final result appears once swelling settles, at about two weeks. Lip filler lasts six to 12 months. Cheek, chin and jawline filler, placed deeper and made of firmer gels, typically lasts 12 to 18 months. Under-eye filler often lasts a year or longer, because that area has little movement. Duration also depends on your metabolism, how much product was placed and how active you are.

Downtime is minimal. Most patients return to work the next day, with swelling that is noticeable to them for two to three days and bruising that can be covered with makeup after 24 hours. Lumps that can be felt but not seen usually soften within two weeks. If a result needs adjustment, hyaluronic acid can be partially or fully dissolved with hyaluronidase, which is rarely needed and always available.

Candidates and safety

Who is a good candidate for fillers

Fillers suit adults who have lost volume in the cheeks, lips or under the eyes, who want a stronger chin or jawline, or who want more lip shape and fullness. Lip fillers are FDA-approved for patients 21 and older, and the practice follows that guidance. Skin quality matters as well: fillers restore volume and structure, and lines caused by skin laxity or sun damage respond better to microneedling, peels or skin boosters, which Tami will recommend when they are the better fit.

Treatment is postponed during pregnancy and breastfeeding, with an active skin infection or cold sore near the area, and within two weeks of dental work. Tell Tami about bleeding disorders, blood thinners, autoimmune conditions, previous filler and any permanent implants in the face. The practice works toward proportionate, natural-looking results and will say so at the consultation when a request falls outside that approach.

How long does lip filler last?

Six to 12 months for most patients. The lips move constantly, so filler there breaks down faster than in the cheeks or chin. A first syringe often lasts closer to six months, and repeat treatments tend to last longer because some product remains from the previous visit.

Are dermal fillers safe?

Hyaluronic acid fillers have been FDA-approved since 2003 and have a long safety record. Common effects are swelling, bruising and temporary tenderness. The serious risk, vascular occlusion, when filler blocks a blood vessel, is rare and is minimized by anatomical knowledge, careful technique, slow injection and the use of cannulas in higher-risk areas. Tami is trained to recognize and treat this complication and keeps hyaluronidase, the enzyme that dissolves filler, available at every visit.

Are dermal fillers permanent?

No. Hyaluronic acid is broken down by the body over six to 18 months, depending on the area and the product. It can also be dissolved on purpose with hyaluronidase if you want to reverse or adjust a result. Rethink does not use permanent fillers.

Does filler hurt?

Most patients describe pressure and a few sharp moments. A topical numbing cream is applied for 15 to 20 minutes, and the fillers contain lidocaine, so the area becomes numb as treatment progresses. The lips are the most sensitive area. The cheeks, chin and jawline are usually very comfortable.

How much swelling should I expect, and when will I see the final result?

Swelling peaks in the first 24 to 48 hours, especially in the lips, and settles within a few days. Bruising, when it occurs, fades within a week and can be covered with makeup after 24 hours. You see the change immediately, and the settled result shows at about two weeks, which is when the follow-up visit is scheduled.
