What Botox and other neuromodulators do
Botox, Dysport, Xeomin, Daxxify and Letybo are neuromodulators: purified forms of botulinum toxin type A that the FDA has approved for cosmetic use. A small dose injected into a specific facial muscle blocks the nerve signal that tells that muscle to contract. The muscle relaxes, the skin above it stops folding in the same place many times a day, and the line softens. Lines that are visible only during expression usually disappear. Lines that have become etched into the skin at rest soften over several treatment cycles, because the skin has time to repair while the fold is at rest.
The effect is temporary and depends on the dose. Tami uses the smallest dose that achieves the result you want, so treated areas look rested and your expressions stay readable. The forehead in particular is dosed conservatively and balanced against the frown lines, since over-treating one area can pull the brows down or lift them too far.

How it works
How Botox treatment works at Rethink
Appointments are one patient at a time in a private treatment suite at 496 Plandome Road. A first visit, including the consultation, takes about 45 minutes. Follow-up treatments take 20 to 30 minutes.

Consultation and assessment
Tami reviews your health history, medications and previous treatments, then watches how your face moves when you frown, raise your brows and smile. Together you agree on the areas, the product and the dose. New patients receive this consultation free of charge, and there is no obligation to treat the same day.

Treatment
The skin is cleansed and the injection points are mapped. Injections are made with a very fine needle, a few seconds each, and most patients describe a brief pinch. Numbing is rarely needed, and ice is available. Ten to twenty small injections cover the frown lines, forehead and crow's feet.

Aftercare
Stay upright for four hours and do not rub or massage the treated areas for the rest of the day. Skip strenuous exercise, saunas and facials for 24 hours. Makeup can go on after a few hours. Small red bumps at the injection points usually fade within 30 minutes.

Follow-up
Results begin within three to seven days and are complete by day 14. A follow-up at about two weeks is offered to new patients, and a small adjustment is added if one side moves more than the other. Notes on dose and placement guide your next visit, usually three to four months later.

Results, how long Botox lasts and downtime
The first change appears three to seven days after treatment, when the treated muscles begin to relax. The full effect is visible at about two weeks. Results from Botox, Dysport, Xeomin and Letybo last three to four months for most patients. Daxxify uses a different stabilizing peptide and lasted about six months in its clinical trials, which makes it an option for patients who want fewer visits a year. If a result wears off unusually fast, the dose or the product is adjusted at the next visit.
There is no downtime. You can return to work or errands immediately. Possible short-term effects include pinpoint bruising, mild swelling at the injection points and, in a small number of patients, a headache for a day or two. Patients who keep a regular schedule often find that the treated muscles weaken over time, so the same result needs a slightly lower dose or lasts longer.
Candidates and safety
Who is a good candidate for injectables
Neuromodulators suit healthy adults who want to soften expression lines, keep them from deepening, slim the jaw or lift the brows slightly. Many patients start in their late twenties or thirties, when lines are still visible only during expression. Treating at this stage, sometimes called preventative or baby Botox, uses smaller doses to keep lines from settling in. Patients in their forties, fifties and beyond see good results on dynamic lines, and deeper static lines improve gradually or are combined with filler or skin treatments.
Treatment is postponed if you are pregnant, breastfeeding or trying to conceive, if you have an active skin infection in the area, or if you have a neuromuscular condition such as myasthenia gravis. Tell Tami about any allergy to botulinum toxin products, recent antibiotics and blood thinners, since these affect timing and bruising risk. Expectations matter as well: neuromodulators do not add volume or lift sagging skin, and Tami will say so when another treatment would serve you better.

How long does Botox last?

Does Botox hurt?
Most patients feel a brief pinch at each injection point, similar to a mosquito bite, and the injections themselves take 10 to 15 minutes. Numbing is rarely needed, and ice is available. Any redness or small bumps settle within about 30 minutes.
What should I avoid after Botox?
Stay upright for four hours, do not rub or massage the treated areas the same day, and skip intense exercise, saunas, hot yoga and facials for 24 hours. Alcohol and blood thinners such as ibuprofen increase bruising during the first day. Makeup can be applied after a few hours.
How many units of Botox do I need for my forehead?

Is Botox safe?

Uncommon effects such as a heavy brow or a drooping eyelid are related to placement and dose, resolve as the product wears off, and are minimized by an experienced injector who knows the anatomy of the face. Treatment is not given during pregnancy or breastfeeding.
