What hyperhidrosis treatment with Botox is

Hyperhidrosis is sweating beyond what the body needs to cool itself. In primary axillary hyperhidrosis, the most common form, the underarm sweat glands are overactive without any underlying illness: shirts are soaked within an hour, antiperspirants labeled clinical strength stop working, and choosing clothes becomes a daily calculation. The condition often starts in the teens, runs in families and responds well to treatment.

Botox (onabotulinumtoxinA) is FDA approved for severe underarm sweating that has not improved with prescription antiperspirants. The medication is placed in a grid of small, shallow injections into the skin of each underarm, where it blocks acetylcholine, the chemical messenger that tells the sweat glands to produce sweat. The glands stay healthy; they stop receiving the signal. In clinical studies sweat production in the treated area fell by more than half in about nine out of ten patients, and by about 80 percent on average. Sweating elsewhere continues as normal, and temperature control is unaffected because the underarms hold a small fraction of the body’s sweat glands.

Consultation and assessment

Tami reviews your sweating pattern, what you have already tried, your health history, medications and allergies, and examines the underarms. She rules out causes that need separate care, such as thyroid disease or a medication side effect, confirms that Botox is appropriate and explains the expected result, how long it lasts and the cost. Photographs are taken for your record.

Preparation

Shave your underarms the day before and skip deodorant and antiperspirant on the morning of your appointment. Avoid exercise, caffeine and hot drinks for the 30 minutes before the visit. In the suite the skin is cleansed and the sweating zone is mapped: where useful, a starch-iodine test darkens the skin exactly where sweat is produced, and Tami marks a grid of injection points about a centimeter apart.

The injections

Using a very fine needle, Tami places about 15 to 20 shallow injections into each underarm, a few seconds apiece, for a standard dose of around 50 units per side. Most patients describe a light pinch; ice or a numbing cream is available if you prefer. The injection stage itself takes about ten minutes.

Aftercare and follow-up

You leave with nothing to cover and can return to work immediately. Leave antiperspirant off until the next morning and skip intense exercise, saunas and hot yoga for 24 hours. Sweating begins to fall within two to four days and reaches its full reduction at about two weeks, when Tami checks in; a small touch-up is added if a spot is still active.

Results

Results, how long they last and side effects

Most patients notice a clear change within the first week and the full effect by two weeks: dry underarms through a workday, a workout or a summer afternoon, with no marks on shirts. Relief lasts four to six months on average, many patients stay dry for seven months or longer, and repeat treatments tend to last at least as long as the first. Two visits a year keep most people dry year round; some choose one treatment in late spring to cover the summer.

Side effects are minor and short. Expect pinpoint marks that fade within hours and occasionally a small bruise or tenderness for a day or two. Some patients notice slightly more sweating elsewhere for a few weeks as the body adjusts; it is usually mild and settles on its own. Because the injections stay in the skin, weakness in the arm is rare and temporary when it does occur. There is no downtime.

Candidates and safety

Who is a good candidate for Botox for sweating

The treatment suits adults whose underarm sweating interferes with clothing, work or social life and who have already tried regular or clinical-strength antiperspirants. It suits constant sweating as well as sweating triggered by stress, heat or exercise, from the late teens onward. Patients with moderate sweating who want dry underarms for an event or a season are also good candidates.

Treatment is postponed with an infection or rash in the underarm and avoided during pregnancy and breastfeeding, with a neuromuscular condition such as myasthenia gravis or ALS, and with a known allergy to botulinum toxin or to the albumin in the formulation. Tell Tami about every medication, especially blood thinners, which raise the chance of bruising, and aminoglycoside antibiotics, which can increase the effect of the toxin. Sweating that started suddenly, occurs mostly at night or affects the whole body needs a medical workup first, and Tami will refer you before treating.
